WHAT IS A MEDICAL HOME?

 

 

A Medical home is a nationally known healthcare standard that is based on a cultivated partnership between the patient, family, and primary provider in cooperation with specialists and support from the community. The patient/family is the focal point of this model. 

Patient Centered Medical Home guidelines stress that care under the medical home model must be accessible, family-centered, continuous, comprehensive, coordinated, compassionate, and culturally effective.  Electronic Health Record systems are used to monitor and track health outcomes in real time. Through a health team approach, practices are transformed to encourage develop improvements in care and provide patients with an increased quality of service.
