Visiting Nurse Association of Central Jersey Community Health Center

B ased on the experience of providing visiting nurse services in the Asbury Park area for more than 80 years, the Visiting Nurse Association of Central Jersey opened this primary care center in 1994 as a response to an identified unmet need for accessible comprehensive high quality health care services. The Community Health Center is open to residents of all ages. A variety of third-party insurance is accepted and sliding scale fees are available. The center is open convenient hours Monday through Saturday, including evening hours. Cultural sensitivity is a hallmark of the center and multiple language translation is available.

As a fully active community partner, the VNACJ Community Health Center extends its educational and health screening services to a variety of settings throughout the community. The door has been open to many of support groups who find hospitality and comfort in this meeting site. The VNACJ Community Health Center is privileged to be a health partner in a community that has recognized the value of this resource. It will continue to be responsive to the community's calls for service.

The VNACJ Community Health Center offers primary and preventative health care services for people of all ages. Care is family-focused and emphasizes health education and illness prevention. This nurse-managed health center promotes healthy lifestyles in order to decrease the potential for illness.

When illness does occur, "first line" primary care helps prevent minor problems from becoming serious health emergencies. The Community Health Center is an innovative model of community-based care that is supported through the partnership and collaboration among the VNA of Central Jersey, Jersey Shore Medical Center and the Monmouth County Health Department. Coordinated and comprehensive quaIity health care is provided through the services of a dedicated health care team. Nurse practitioner, physicians, and community health nurses work to develop a balance of health care approaches that look toward prevention and partnership as well as direct intervention. Housed in the same service area as the regional office of the Visiting Nurse Association of Central Jersey, the Community Health Center is able to access the services of the neighborhood nurses for home evaluations and direct services.
