MEET THE PRESENTERS

 

Thomas R. Ortiz, MD, FAAFP

NJ Innovations Institute for Health Care

 

 

Dr. Thomas R. Ortiz, a board certified Family Physician, is the Founder and Medical Director of Forest Hill Family Health Associates, PA (www.fhfha.com) in Newark, NJ since 1984. In 2010 his practice was nationally recognized by the NCQA as a Patient-Centered Medical Home.

 

He is a graduate of New Jersey Medical School in Newark, NJ. Dr. Ortiz joined the National Health Service Corp. and has made a career as a public health advocate for universal access to primary care for all, focused on the poor and vulnerable populations.A longtime champion for healthcare justice.

 

Most Recently, Dr. Ortiz has been named the Medical Director of the NJ Innovations Institute for Health Care, CMS funded Garden Practice Transformation Network, tasked with bringing practice transformation support services into value-based alternative payment models to primary care practitioners.

Ilse Bell, MD, PCMH-CCE

Dr. Ilse Bell is a results-oriented healthcare professional who engages physician leadership and drives practice transformation to team-based patient-centered care. Her application of NCQA PCMH recognition standards has resulted in    numerous multi-site primary care practices achieving NCQA PCMH Level 3 recognition.   

During her 7 years at Amerigroup Community Care, NJ (Anthem), she trained practices in  quality improvement and cost containment metrics; benchmarked results; and created practice transformation procedures and tools to accelerate the advancement of alternative
payment models across the state. She also served as the health plan liaison for the implementation of the Comprehensive Primary Care Initiative (CPCi) and the Comprehensive Primary Care Plus (CPC+) in the New Jersey market. Ilse received her doctor of medicine degree from Brown University and her BS in biology from Spelman College.
