NJPCA is pleased to announce that New Jersey health centers have received a total of 21 Health Center Quality Improvement Awards totaling $1,882,596 for the 2016 fiscal year. A complete list of the various awards and recipients is provided below:

EHR Reporter Awards: were awarded to centers that employed EHRs to report on all clinical quality measures (CQM) data for all of the health center’s patients. Nine (9) health centers received a total of $90,000. The recipients are:

National Quality Leader Awards: this award was given to center that exceeded national clinical quality benchmarks, including Healthy People 2020 goals for chronic disease management, preventive care, and perinatal/prenatal care. The following 4 centers received a total of $138,970:

Access Enhancers Awards: health centers that increased the total number of patients served and the number of patients receiving comprehensive services between 2014 and 2015 were awarded the Access Enhancers Awards. Seven (7) centers in New Jersey (listed below) received $55,000:

High Value Health Centers Awards: health centers that delivered better, smarter, healthier care by improving cost efficient care delivery compared to the national average while also increasing quality of care and improving access to comprehensive services received this award. Four centers in New Jersey received a total of $100,000 and these centers are:

PCMH Recognition Awards: – these awards were given to health centers that achieved PCMH recognition for their service delivery sites. Thirteen (13) health centers in New Jersey received $620,000 as PCMH Recognition Awards. The recipients are:
