HealthEfficient and New Jersey Primary Care Association Announce Strategic Partnership to Strengthen Data-Driven Care Across New Jersey

Hamilton, New Jersey — HealthEfficient, a nationally recognized non-profit organization and federally-designated Health Center Controlled Network (HCCN), and the New Jersey Primary Care Association (NJPCA) today announced a new strategic partnership to support community health centers across New Jersey in advancing clinical quality, operational performance, and data-driven care.

Through this collaboration, NJPCA will work alongside HealthEfficient to expand engagement in HealthEfficient’s federally funded HCCN, connecting New Jersey health centers to a national network of more than 65 member organizations focused on leveraging Health Information Technology to improve patient outcomes.

“This partnership reflects our shared commitment to strengthening the capacity of New Jersey’s community health centers,” said Alan Mitchell, Executive Director of HealthEfficient. “By working closely with NJPCA, we can better align national resources with local needs and accelerate meaningful improvements in care delivery.”

The agreement supports HealthEfficient’s HCCN program through July 2027 and focuses on outreach, engagement, coordination, and targeted practice facilitation support for participating health centers, including HealthEfficient’s cohort of 11 participating health centers in New Jersey.

Advancing Key Priorities for Community Health Centers

The partnership is designed to help health centers address critical priorities, including:

  • Using data and analytics to improve clinical quality and operational performance
  • Enhancing interoperability and health information exchange
  • Expanding participation in value-based care models
  • Strengthening the identification and response to patients’ health-related social needs
  • Leveraging modern technologies, including FHIR-based applications and advanced analytics

HealthEfficient brings deep expertise in electronic health record optimization, data strategy, and performance improvement, while NJPCA contributes longstanding relationships with New Jersey health centers and a strong understanding of local needs and priorities.

A Collaborative, Practice-Focused Approach

Under the partnership, NJPCA will support:

  • Recruitment and onboarding of participating health centers
  • Ongoing engagement and coordination across members
  • Facilitation of peer learning and collaboration
  • Alignment of HCCN activities with statewide priorities

HealthEfficient will provide technical assistance, implementation support, and national best practices drawn from its work across multiple states and regions, including the Pacific Islands.

The partnership also creates new opportunities for peer learning among New Jersey health centers and across HealthEfficient’s national network. Participating organizations will be able to share best practices, learn from high-performing peers, and collaborate on solutions to common challenges.

By aligning NJPCA’s statewide leadership with HealthEfficient’s national capabilities, the partnership establishes a strong foundation for sustained improvement in health center performance and patient outcomes.
