NJPCA partnered with Weitzman Institute to host the Clinical Directors Peer Support Work Group training, “Integrating Social Risk Factors in Hypertension Prevention.” This virtual training was conducted on February 7, 2024 from 11:00AM to 12:30PM. The training featured a presentation from Mary Blankson DNP, APRN, FNP-C, FAAN, Chief Nursing Officer from Community Health Center, Inc. The training focused on explaining how social determinants of health affect patient outcomes, focusing on key factors related to hypertension. It also discussed how to utilize the care team to address patients' social needs and use knowledge of social risk factors to create personalized hypertension care plans.
