The session, “The Social Determinants of Health: Informed Policies into Clinical Care and Management”, was presented by Dr. Kevin Lombardi, MPH, Manager of Policy, Research, and Health Promotion, The National Center for Health in Public Housing (NCHPH). It discussed a conceptual overview of the Social Determinants of Health as well an outline of key recommendations and promising practices regarding the integration of SDOH-informed policies into patient care.
